DSS-Approved ABI & Elder Care Services

Butler Rescue Hands, Inc. is an approved provider with the Connecticut Department of Social Services (DSS), delivering home- and community-based services under both Acquired Brain Injury (ABI) Waiver Programs and Elder Care programs. Our services are person-centered, goal-driven, and designed to support independence, safety, and community integration.


ACQUIRED BRAIN INJURY (ABI) SERVICES

We provide structured, rehabilitative, and supportive services to adults living with traumatic or non-traumatic brain injuries, including stroke, aneurysm, hypoxia, tumor, and neurological conditions.

Independent Living Skills Training (ILST)

ILST is a goal-oriented service focused on helping individuals develop, regain, or maintain skills necessary for independent living.

ILST includes training and support in:
• Personal care routines and hygiene
• Medication reminders and daily scheduling
• Meal planning, cooking, and nutrition
• Household management and home safety
• Budgeting, money management, and bill support
• Time management and task completion
• Memory strategies, cueing, and compensatory techniques
• Community access and transportation training
• Communication, problem-solving, and decision-making skills


Residential Assistance Level 1 (RA1)

RA1 provides intermittent, non-continuous support for individuals who require structured assistance but do not need 24-hour supervision.

RA1 services include:
• Cueing and prompting for daily routines
• Supervision to ensure health and safety
• Assistance with household tasks
• Support with scheduling and appointments
• Skill reinforcement related to ILST goals
• Monitoring to promote independence


Residential Assistance Level 2 (RA2)

RA2 provides more intensive, ongoing supervision and assistance for individuals requiring higher levels of support.

RA2 services include:
• Extended supervision and safety monitoring
• Hands-on assistance with daily living activities
• Behavioral support and redirection
• Routine development and structure
• Crisis prevention and de-escalation support
• Coordination with care teams and case managers


Personal Care Assistance (PCA – ABI)

PCA services provide hands-on assistance with activities of daily living for ABI participants.

PCA services include:
• Bathing, grooming, and personal hygiene
• Dressing and toileting assistance
• Transfers and mobility support
• Fall-prevention and safety monitoring
• Cueing and redirection related to cognitive needs


Supported Employment (ABI)

Supported Employment assists individuals with ABI in preparing for, obtaining, and maintaining meaningful employment.

Services include:
• Job readiness and skill development
• Resume building and interview preparation
• Workplace behavior and communication support
• On-the-job coaching and skill reinforcement
• Coordination with employers and vocational partners
• Support for job retention and long-term success


Community Integration & Skill Building (ABI)

• Accompaniment to appointments and activities
• Social skill development and engagement
• Support participating in community programs
• Building independence in public settings
• Reintegration into work, volunteer, or day programs


ELDER CARE SERVICES (DSS & Allied Programs)

Our Elder Care services support adults 65 and older who require assistance to safely remain at home and avoid institutional care.


Personal Care Assistance (PCA – Hourly)

Hourly PCA services support seniors with daily self-care needs while promoting dignity and independence.

Services include:
• Bathing, grooming, and hygiene
• Dressing and toileting assistance
• Transfers and mobility support
• Medication reminders
• Fall-prevention and safety monitoring


Personal Care Assistance (PCA – Live-In)

Live-In PCA provides continuous in-home support for individuals requiring ongoing assistance.

Services include:
• 24-hour presence and supervision
• Assistance with ADLs throughout the day
• Overnight safety monitoring
• Routine support and structure
• Peace of mind for families


Transportation Services

Transportation services support access to essential community resources.

Includes transportation to:
• Medical appointments
• Therapy and treatment services
• Grocery shopping and errands
• Community and social activities


Homemaker / Homemaking Services

Homemaking services support a safe, clean, and healthy living environment.

Services include:
• Light housekeeping
• Laundry and linen changes
• Meal preparation and clean-up
• Grocery shopping and errands
• Home organization and safety maintenance


Companion Services

Companion services provide non-medical supervision and emotional support.

Services include:
• Companionship and conversation
• Social engagement and emotional support
• Safety supervision
• Cognitive stimulation and daily routines
• Support reducing isolation and loneliness


CARE COORDINATION & COLLABORATION

Across all programs, we provide:

• Ongoing communication with DSS case managers
• Coordination with families and authorized representatives
• Documentation aligned with waiver requirements
• Service monitoring and plan adjustments
• Advocacy and continuity of care
